Individual Operator
You hold the franchise yourself and are collecting in person.

Representative
You are collecting on behalf of the operator.

Special Power of Attorney (SPA)
Executed by the operator, authorising you to claim on their behalf.

Operator's Valid Government-Issued I.D. Card
The ID of the OPERATOR who signed the SPA, not your own.
Any one of the accepted IDs listed below.
Cooperative
The franchise is held by a cooperative or corporation.

Board Resolution
The resolution authorising the release and naming who may claim it.

Secretary's Certificate
Certifying the board resolution above.

Valid Government-Issued I.D. Card
Any one of the accepted IDs listed below.

Wherever an ID is asked for above, any ONE of these is accepted.
- PhilSys ID — PSA
- Driver's License — LTO office
- Passport — DFA Office of Consular Affairs
- UMID / GSIS e-card / SSS e-card — GSIS or SSS office
- PRC ID — Professional Regulation Commission
- IBP ID — Integrated Bar of the Philippines
- Pag-IBIG ID — Pag-IBIG office
- Senior Citizens' ID — Local Government Unit (OSCA)
- Voter's ID — COMELEC office
- Postal ID — Philippine Postal Corporation
Bring the original documents. Staff examine them at the window; nothing needs to be uploaded beforehand.
